Pre-Employment

Employment Forms and Instructions

Read these instructions completely before you start.

Be sure that each section is completed in accordance with the instructions. Do not start until you have read and understood the instructions for completing the forms. If you need help, call (972) 647-2000 Monday through Thursday, 9:00 A.M. until 4:00 P.M. If you call after hours, the Dispatcher on Duty will NOT be able to help you.

1. Download and Print the forms using

bullet Able Driver Staffing Pre-Employment Application (eleven pages)
bulletUSIS/DAC Drug and Alcohol Release (one page)
bulletInternal Revenue Service Form W4 (two pages)
bullet Immigration and Naturalization Form I-9 (three pages)

These forms are in PDF format, and require that you download Acrobat.

 

2. Pre-Employment Application

bulletPage 1:    Read this page carefully, be sure you understand each item, sign and date at the bottom.
bulletPage 2.    Complete all information. Be sure you include your residence address(es) for the last three (3) years.
bulletPage 3/4  Complete all information. Make sure there is a contact name and telephone number for each position you have held. Print additional copies of this page if you need more space to complete ten (10) years of employment history, as required by DOT Regulation. Applications that do not include ten years of employment history will NOT be considered, unless you were a student or unemployed during part of the ten years. For periods when you were a student, write "Student," state the name and location of the Educational Institution you attended, and define the dates you attended. For periods of unemployment, write "Unemployed" and define the dates of your unemployment.
bulletPage 5.    State the dates and reasons for any employment gaps of 30 days or greater. List your military experience. Sign and date.
bulletPage 6.    Complete all information. Be sure to include all accidents in which you have been involved, regardless of fault or whether you were in a commercial vehicle, as required by Regulation. Include all traffic convictions and forfeitures (except parking citations) regardless of whether or not you were in a commercial vehicle. Include all unresolved citations (citation issued but no finding by the court). Provide complete details of any Workman's Compensation Claims, including dates, injuries, and circumstances of the accident(s). Fill out the remainder of the form.
bulletPage 7/8    Read these pages carefully, sign your name, print the date, print your name and print your Social Security number.
bulletPage 9.    Fill out the first two lines of this page with your name, Social Security Number, and your signature. DO NOT fill in any other portion of this page.
bulletPage 10    Sign this form in the upper right hand corner. DO NOT fill in any other portion of this form.
bulletPage 11    Fill in your name on the first line. DO NOT fill in any other portion of this form.

3. USIS/DAC DOT Drug and Alcohol Release

Fill in your printed name and signature in two places on the form, enter your Social Security number and the date where indicated.

3. IRS Form W-2 and INS Form I-9

Complete these forms in accordance with the printed instructions that are included with the forms.

 

4. Copies of your Driver's License, DOT Card (Med Card), and Social Security card

Make copies of your Driver's License, your DOT Card, and your Social Security Card.  (We will make the copies if you come to our office in person).

 

5. Submit the completed forms and copies

Bring the completed forms to our office in Grand Prairie Monday through Thursday, 9:00 A.M. until 4:00 P.M. If you are out of the area, FAX the completed forms and the copies to (972) 602-7177.  Call (972) 647-2000 to make sure we received your FAX.
